Why Your MRI Doesn't Always Explain Your Pain
You've finally had an MRI. You open the report and see words like disc bulge, arthritis, degeneration, rotator cuff tear, or meniscus tear. Immediately, your mind jumps to the worst-case scenario. "No wonder I'm in pain." But here's something that surprises many people: an MRI doesn't always tell the whole story.

While MRIs are excellent tools for identifying structural changes, they don't always explain why someone is experiencing pain—or why someone else with the exact same MRI findings feels perfectly fine.
Research has shown that many healthy adults with no pain at all have MRI findings such as disc bulges, arthritis, tendon tears, and cartilage changes. These changes are often a normal part of aging, much like wrinkles on your skin or gray hair. In many cases, they're simply part of life—not necessarily the source of your symptoms.
On the other hand, someone with significant pain may have an MRI that looks relatively normal. That's because pain is influenced by much more than what appears on an image. Your strength, mobility, movement patterns, previous injuries, activity level, sleep, stress, and overall health all play a role in how your body feels and functions.
This is why treating an MRI instead of treating the person can lead to frustration. If all the attention is placed on the imaging report, the underlying cause of the problem may be overlooked.
Take shoulder pain, for example. An MRI may reveal a partial rotator cuff tear, but the real issue could also involve poor shoulder blade control, weakness, limited thoracic spine mobility, or faulty movement mechanics. Likewise, someone with low back pain may have a disc bulge on imaging, but their symptoms could be driven by reduced hip mobility, poor core stability, or prolonged sitting rather than the disc itself.
